Customer Acknowledgment — Read this document in its entirety before operating any scooter.
This waiver and the accompanying Safety & Operations Manual use the following signal words. All riders must understand these designations before operating a scooter.
The following safety rules apply to all mobility scooter models. Every operator must review and acknowledge these rules before using any scooter.
Starting: Insert key into the tiller key switch and turn to ON. Set the speed dial to the slowest (tortoise) setting.
Moving Forward: Pull the right side of the accelerator lever toward you. The further you pull, the faster you go.
Reversing: Pull the left side of the accelerator lever toward you (or push the right side away). Reverse speed is slower than forward speed.
Stopping: Simply release the accelerator/throttle lever. The scooter will slow gradually and the brakes engage automatically.
Approach unfamiliar doors slowly. Determine whether the door opens toward or away from you. If it opens away, drive through and allow a self-closing door to close behind you. If it opens toward you, hold the handle while gently pressing the reverse lever to open it, then switch to forward once the door is wide enough. In tight spaces, stop completely and then apply power gently while turning.
4.1 Charge batteries daily when the scooter is in regular use. Charge for 8–14 hours in a dry location. Extended charging is safe and will not overcharge the batteries; deep discharge can damage them.
4.2 The battery meter uses a color code: green = fully charged, yellow = draining, red = immediate recharge needed.
Power-operated vehicles may be susceptible to electromagnetic interference from radio stations, TV stations, amateur radio transmitters, two-way radios, and cellular phones. EMI can cause the scooter to release its brakes, move by itself, or move in unintended directions, and may permanently damage the control system.
6.1 Most devices resist light moisture but are not waterproof. Avoid heavy rain and dry the device after any exposure. Do not ride the scooter while it is wet.
6.2 Extreme heat drains batteries faster — park in shade when possible. Cold weather reduces battery performance — charge indoors when possible.
All models include a manual brake release lever that allows the scooter to be pushed without power. The lever is located at the top right of the rear section.
9.1 A main circuit breaker (reset button) is located on the battery pack. If the breaker trips, wait 1–2 minutes, then press the reset button. If it trips frequently, the batteries may need a load test by an authorized provider.
9.2 If you notice jerking movement, sudden power loss, or brake delay — stop using the device immediately and contact Folsom Mobility Scooter, LLC.
10.1 By signing this waiver, I acknowledge that I am voluntarily operating a mobility scooter provided by Folsom Mobility Scooter, LLC. I understand that operating a mobility scooter involves inherent risks, including but not limited to physical injury, tipping, collision, property damage, or other hazards.
10.2 In consideration of being permitted to operate a scooter, I hereby release, waive, and discharge Folsom Mobility Scooter, LLC, its officers, directors, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained during operation.
10.3 I agree to indemnify and hold harmless Folsom Mobility Scooter, LLC from any loss, liability, damage, or cost incurred due to my operation of the scooter, including reasonable attorney fees and court costs.
10.4 I authorize Folsom Mobility Scooter, LLC to seek emergency medical treatment on my behalf if necessary. I understand that any medical costs incurred will be my sole responsibility.
